1. ¿Qué se afirmó?

85% improvement in 5–10 days after stopping bread; complete elimination of reflux and IBS.

2. ¿Qué es demostrablemente correcto?

Some patients improve with dietary change (see C-GUT-01).

3. ¿Qué es erróneo o exagerado?

The 85% figure has no identified source. The statement that both diseases can be 'eliminated completely' through habits contradicts guidelines that recommend drugs, psychotherapy and other measures because diet alone often does not suffice.

4. ¿Qué sigue sin estar claro?

Elawady's clinical experience is not documented in a published, controlled form.

5. ¿Qué estudios lo respaldan?

None.

6. ¿Qué estudios lo contradicen?

ACG IBS 2021 (I-GUT-05) and ACG GERD 2022 (I-PPI-10) recommend multiple treatment options beyond diet. Gluten-free diet RCT: 58% responders at 4 weeks (I-GUT-103); blinded wheat vs sham challenge: no significant difference (39% vs 29% worsening; I-GUT-120); IBS placebo response 27.3% (I-GUT-104); diet NMA (I-GUT-102); lifestyle explains about 37% of GERD symptoms in a large cohort (I-GUT-105). NICE NG12 and CG184 require investigation of alarm symptoms (I-GUT-109, I-GUT-108).

7. ¿Qué se aplica a Alemania y Europa?

Not applicable.

8. ¿Qué se sabe sobre Egipto?

Not applicable.

No source for the 85% figure exists. Randomised data now contradict it directly: in the closest trial a gluten-free (wheat-free) diet gave 58% responders after 4 weeks (95% CI 41%-73%; I-GUT-103), a network meta-analysis of 28 RCTs shows diets help a proportion of IBS patients, not nearly all (I-GUT-102), a blinded wheat challenge was not worse than sham in IBS patients who believed wheat harmed them (I-GUT-120), and placebo responses of 27-34% in IBS trials show why uncontrolled success rates are inflated (I-GUT-104). Certainty is moderate because no trial tested the exact regimen.

The 85%-in-10-days figure and the promise of complete elimination are not supported; diet helps a part of patients over weeks, and guidelines require investigation of alarm symptoms.

Por qué algunos estudios tienen más peso

Randomised and blinded data and placebo-response data outweigh unpublished clinical impressions; no source for the 85% figure was found.

¿Está demostrada una relación causal? No demostrada No controlled study tested the specific regimen; the best related RCT shows lower response rates and blinded data attribute much of perceived wheat effects to expectation.

Incertidumbre restante

Response of Egyptian patients to bread removal was never studied.

Conclusión justificada

The quantitative promise is unsupported and the 'complete elimination' statement contradicts guidelines; diet is one part of care.

¿Por qué parecen contradecirse los estudios?

No real conflict: supportive data show partial response, not cure; the claim's figure appears to derive from uncontrolled observation.
