Mostly false Certainty of evidence: low provisional C-CHM-11

Cancer cells resist chemotherapy because of their fibrotic interstitial (stromal) environment; fibrosis strangles cells, which then turn cancerous and resist chemo and radiation.

Safety note: Do not refuse, stop or delay recommended cancer treatment (surgery, chemotherapy, radiotherapy, hormone or targeted therapy) in favour of a diet. In curable cancers, replacing standard treatment with alternative approaches is associated with a markedly higher risk of death (I-CHM-03, I-CHM-04), and even four-week delays are associated with higher mortality (I-CHM-07). Late effects are real and are managed by survivorship and cardio-oncology care; discuss them with the treating oncology team. Nutrition can be adapted alongside treatment with the oncology team. No individual medical advice is given here.
Conclusion: Stroma can contribute to drug resistance in some tumours, but it is not a general explanation and does not justify avoiding treatment.

Scientific assessment

Mostly false – Essential parts of the statement are not supported by the evidence or are strongly exaggerated.

Certainty of evidence: low – Further research is likely to change the assessment considerably.

1. What was claimed?

Chemoresistance is caused by the fibrotic interstitial environment; fibrosis creates cancer.

2. What is demonstrably correct?

Dense tumour stroma can impair drug delivery: in a genetically engineered mouse model of pancreatic cancer, only 32% of tumour vessels were perfused; depleting the stroma raised intratumoral gemcitabine metabolites by 60% and transiently prolonged survival (11 to 25 days) before tumours resumed growth (I-CHM-11). This supports only that tumour stroma can hinder drug delivery in one tumour type in mice. Chronic inflammation/fibrosis is a recognised risk setting for some cancers (not reviewed in detail here).

3. What is wrong or exaggerated?

As a universal rule it is wrong: many cancers are highly chemosensitive and curable (childhood ALL 5-year survival 90.4%, I-CHM-05; testicular cancer 95% 10-year survival, A-CHM-14). Resistance has multiple causes (tumour genetics, drug efflux, etc.). The step from 'resistance exists' to 'do not treat' is not supported.

4. What remains unclear?

The share of chemoresistance attributable to stroma in human cancers. No source reviewed here tests the claim that fibrosis causes malignant transformation or radiation resistance.

5. Which studies support it?

I-CHM-11 (partial: stroma hinders drug delivery in mouse pancreatic cancer; does not test fibrosis as a cause of cancer or as a general explanation of chemoresistance).

6. Which studies contradict it?

I-CHM-05, A-CHM-14 (indirect: both document high chemosensitivity and curability of some cancers; neither tests the fibrosis hypothesis itself).

7. What applies to Germany and Europe?

Not region-specific: tumour biology and chemosensitivity do not depend on the country; no Europe-specific data were reviewed.

8. What is known about Egypt?

Not region-specific; no Egyptian data were reviewed. Differences in access to treatment do not change the biology.

9. What practical conclusion is appropriate?

Resistance to one drug is a reason to choose other treatments with the oncologist, not to stop treatment.

Assessment details

Rationale for certainty of evidence

Mechanistic animal data support a partial stromal contribution in some tumours; as a general explanation it conflicts with the high chemosensitivity of many cancers. Evidence base reviewed here is limited.

In plain language

Some tumours, like pancreatic cancer, are surrounded by dense tissue that can keep drugs out. This is one of several reasons why some cancers respond poorly. But many cancers, such as childhood leukaemia and testicular cancer, respond very well to chemotherapy and are usually cured.
