Overview

Haugen BR et al.. Thyroid 2016 · PMID 26462967 · DOI 10.1089/thy.2015.0020

Most benign thyroid nodules can simply be monitored, but surgery is recommended for confirmed cancer and for large or compressing nodules.

Does the study support the specific claim? Partly · Methodological quality: high

5. Actual results

Results

Selected recommendations read in the PMC full text: no recommendation for or against ultrasound screening even in familial risk (insufficient evidence; Rec 1); FNA is the procedure of choice when indicated (Rec 7); benign nodules are followed by ultrasound according to risk pattern (Recs 23-24); surgery 'may be considered' for growing benign nodules > 4 cm or with compressive symptoms, and most asymptomatic nodules with modest growth should be followed without intervention (Rec 27, strong); asymptomatic cystic nodules may be followed conservatively (Rec 28); for cancers > 4 cm, extrathyroidal extension or metastases, near-total/total thyroidectomy is recommended (Rec 35A, strong).

Authors' conclusion

Evidence-based recommendations representing contemporary optimal care for thyroid nodules and differentiated thyroid cancer.

  • Systematic evidence grading
  • Widely used internationally

9. Assessment of the statistical interpretation

Not applicable.

10. Relevance to the specific claim

Directly addresses the claim's example (thyroid nodules in a working gland).

12. Transferability to Germany and Europe

Used widely in Europe alongside ETA guidance.

13. Overall methodological judgement

Agrees that most benign, asymptomatic nodules should not be operated on; disagrees with never removing a thyroid, because surgery is recommended for confirmed cancers and compressive nodules.
