Valutazione scientifica
1. Che cosa è stato affermato?
Acid blockers are poison and cause cancer, autoimmune disease, infertility, ED, depression, fatty liver and IBS.
2. Che cosa è dimostrabilmente corretto?
Some cohort studies report associations of long-term PPI use with gastric cancer (Cheung 2018: HR 2.44 after H. pylori eradication; Abrahami 2022: HR 1.45 versus H2 blockers, I-PPI-108) and dementia (Gomm 2016: HR 1.44); PPIs are overused (see C-PPI-11).
3. Che cosa è sbagliato o esagerato?
'Deadly poison' and the list of diseases are not supported. The gastric cancer signal is small in absolute terms (about 1 extra case per 1,200-2,100 users over 5-10 years in I-PPI-108) and partly explained by protopathic bias (I-PPI-107); the dementia signal was not confirmed by the COMPASS RCT. No evidence was found for infertility (semen quality unaffected, I-PPI-109), erectile dysfunction, depression, fatty liver or autoimmune disease as PPI effects. Rising cancer or ED figures since 2004 are an ecological correlation without causal value.
4. Che cosa resta non chiaro?
Long-term (>10 years) effects on gastric cancer in specific groups remain under study.
5. Quali studi la supportano?
A-PPI-11 (gastric cancer association), A-PPI-04 (dementia association), I-PPI-108 (gastric cancer HR 1.45 vs H2RA, small absolute risk).
6. Quali studi la contraddicono?
COMPASS (I-PPI-01): no significant differences for cancer, dementia and other outcomes except enteric infections. Sawaid 2026 (I-PPI-107): no excess upper GI cancer with long-term use after handling protopathic bias. Keihani 2018 (I-PPI-109): no association with semen quality.
PPIs are among the most widely prescribed drugs in Germany and Europe; several (e.g. pantoprazole, omeprazole) are sold over the counter in small packs for short-term use. European data (Stockholm, UK GPRD/CPRD, Rotterdam) are among the cited cohorts, so the observational findings apply to European users with the same confounding limits.
8. Che cosa si sa sull’Egitto?
Elawady describes PPIs (Controloc, Nexium) as widely used and sold without prescription in Egypt. No Egypt-specific outcome data were reviewed. The biology and trial results are not expected to differ by country; H. pylori infection, which is common in many countries, is an additional reason why acid-suppression and eradication therapy are prescribed.
The best active-comparator cohort found a gastric cancer HR of 1.45 versus H2RAs with a very small absolute excess (number needed to harm 2121 at 5 years and 1191 at 10 years; I-PPI-108). A 2026 case-control study showed that upper GI cancer associations are concentrated in the months before diagnosis and disappear after accounting for symptoms that lead to PPI prescriptions (protopathic bias; I-PPI-107). The COMPASS RCT found no excess of cancer or other outcomes except enteric infections over 3 years (I-PPI-01). For infertility, the only identified study found no effect on semen quality (I-PPI-109); for erectile dysfunction, depression, autoimmune disease and fatty liver no adequate evidence was found. Ecological trends since 2004 cannot separate PPIs from ageing, diagnosis and other changes.

Apart from a small, uncertain gastric cancer signal, the listed harms are not supported; 'deadly poison' and an explanation of population-wide disease trends are incompatible with randomised and absolute-risk data.
Perché alcuni studi pesano di più
RCT and bias-controlled observational data outweigh crude associations; ecological trend arguments have no causal value.
È accertato un rapporto causale? Non accertato Gastric cancer: possible small effect; other listed diseases: no evidence identified (infertility contradicted weakly; ED, depression, autoimmune disease, fatty liver: no good studies found).
Incertezza residua
Long-term (>10 years) gastric cancer risk in H. pylori-exposed people.
Conclusione giustificata
Mostly false.
Perché gli studi sembrano in disaccordo?
Abrahami (HR 1.45) vs Sawaid (no excess after handling protopathic bias): differ in comparator (H2RA vs non-users), exposure timing and adjustment for symptom diagnoses.
Dipende dal paese?
- Effetti biologici generali
- Pharmacology of PPIs is the same in Egypt and Europe.
- Abitudini alimentari
- Not decisive.
- Origine dell’alimento
- Not relevant.
- Contaminanti
- Not relevant (except ranitidine NDMA impurities, C-PPI-08).
- Condizioni di produzione nazionali
- Generic products are used in both regions; no evidence of different pharmacological effects.
- Norme giuridiche
- PPIs are partly available without prescription in Egypt and (small packs, short-term) in the EU; prescribing practices differ.
- Dati di monitoraggio effettivi
- No Egyptian outcome data identified; one Egypt/Iraq pharmacist survey (I-PPI-120).
Ricontrollo statistico
- Absolute excess from NNH – 47 per 100,000 at 5 y; 84 per 100,000 at 10 y ricalcolato
- Log-symmetry and protopathic pattern – Recent aOR 4.01 (3.20-5.03) vs remote 0.62 (0.51-0.75) consistent CIs; pattern of reverse causation ricalcolato
Certezza dell’insieme delle prove (GRADE, semplificato)
- Esito principale
- Cancer and other serious disease caused by PPIs
- Livello di partenza
- alta (studi randomizzati)
- Motivi di declassamento
- imprecision (3-year RCT)
- indirectness for long-term cancer
- Risultato
- moderata

Ultima revisione scientifica: 26 ottobre 2026
Cronologia del giudizio
- 25 ottobre 2026 · Fase 1 · Valutazione iniziale: Prevalentemente falso (bassa)
- 26 ottobre 2026 · Fase 4 (analisi sistematica delle prove) · Giudizio modificato: Prevalentemente falso (bassa) → Prevalentemente falso (moderata)
Certainty raised: absolute-risk data from the best active-comparator cohort and a study demonstrating protopathic bias now quantify the cancer signal as small and partly artefactual; with the RCT this gives moderate certainty that the sweeping claim is mostly false.
Nuove fonti: I-PPI-107 I-PPI-108 I-PPI-109
In parole semplici

Long-term PPI use linked with a small rise in stomach cancer
Abrahami D et al.. Gut 2022 · PMID 34226290 · DOI 10.1136/gutjnl-2021-325097Studio di coorteverifica incrociata indipendenteabstract esaminato★ studio chiaveCompared with H2 blockers, PPI users had a 45% higher relative risk of stomach cancer, but only about 1 extra case per 1,200-2,100 users over 5-10 years.
Gastric cancer HR 1.45 vs H2RA; absolute excess ~1 per 1000-2000 users

Lo studio spiegato in modo semplice
- Che cosa volevano scoprire i ricercatori?
Compare stomach cancer risk of PPI and H2RA users.
- Come è stato condotto lo studio?
Large UK cohort.
- Quante persone, animali o campioni sono stati studiati?
Over 1.1 million new users.
- Che cosa è stato effettivamente rilevato?
HR 1.45; dose-response.
- Quanto era grande l’effetto osservato?
NNH 2121 (5 y), 1191 (10 y).
If real, the extra risk is small; use PPIs only when indicated.
- Quali sono i limiti dello studio?
Observational; H. pylori not fully known.
Partly supports a cancer concern; contradicts 'deadly poison'.
UK data, applicable to Europe.
Cannot prove causation.

Contesto regionale
Germania / UE
PPIs are among the most widely prescribed drugs in Germany and Europe; several (e.g. pantoprazole, omeprazole) are sold over the counter in small packs for short-term use. European data (Stockholm, UK GPRD/CPRD, Rotterdam) are among the cited cohorts, so the observational findings apply to European users with the same confounding limits.
Egitto
Elawady describes PPIs (Controloc, Nexium) as widely used and sold without prescription in Egypt. No Egypt-specific outcome data were reviewed. The biology and trial results are not expected to differ by country; H. pylori infection, which is common in many countries, is an additional reason why acid-suppression and eradication therapy are prescribed.

Valutazione scientifica generale
