Valutazione scientifica
1. Che cosa è stato affermato?
Ghee and butter do not contribute to arterial disease.
2. Che cosa è dimostrabilmente corretto?
At usual population intakes, butter was only weakly associated with mortality (RR 1.01 per 14 g/day) and not with CVD (RR 1.00) in a meta-analysis of cohorts (I-FAT-03). This estimate is limited: it rests on only 4 cohorts for CVD (3 for CHD), some adjusted for blood cholesterol (a mediator), at mostly modest intakes (median 4.5-46 g/day); no ghee study exists, and the authors rate unsaturated oils as healthier than butter. Some researchers argue that food matrix matters and that some saturated-fat-rich foods (e.g. whole-fat dairy) are not associated with CVD (I-FAT-07). No single food 'directly clogs' arteries.
3. Che cosa è sbagliato o esagerato?
Butter and ghee are rich in saturated fat (general food-composition knowledge). Reducing saturated fat lowers serum cholesterol and reduced combined cardiovascular events by 17% (RR 0.83, 0.70-0.98; moderate-quality evidence) in long-term RCTs (I-FAT-01). LDL causes atherosclerotic cardiovascular disease (I-FAT-02). WHO strongly recommends limiting saturated fat to 10% of energy and replacing it with polyunsaturated fat (I-FAT-08). In the system, ghee is the default cooking fat without quantity limits, which can lead to high saturated-fat intakes.
4. Che cosa resta non chiaro?
Effects of ghee specifically (few studies); effects at moderate intakes within an otherwise healthy diet.
5. Quali studi la supportano?
I-FAT-07 (critical review of saturated-fat limits); partly I-FAT-03 (near-null butter associations at modest intakes); I-FAT-104 (cohort meta-analysis: saturated fat not associated with CHD, RR 1.06, 0.95-1.17, very low certainty); I-FAT-103 (Isfahan cohort, Iran: ghee not associated with CVD events; limited power); I-DRY-101 (2026 US guidelines list butter as an option, while keeping the 10% saturated-fat cap). None of these can show that ghee or high butter intakes are safe for arteries.
6. Quali studi la contraddicono?
I-FAT-01 (Cochrane RCTs), I-FAT-02 (LDL causal), I-FAT-08 (WHO guideline), I-FAT-102 (RCT: replacing ghee with rapeseed oil lowered LDL, triglycerides and liver fat), I-FAT-111 (RCT meta-analysis: butter raised LDL by 0.19 mmol/L compared with the same fat as cheese), I-FAT-101 (US cohorts: highest butter intake HR 1.15 for mortality; replacing 10 g/day butter with plant oils HR 0.83).
WHO and European guidelines recommend limiting saturated fat (I-FAT-08).
8. Che cosa si sa sull’Egitto?
Ghee is traditional in Egyptian cooking; cardiovascular disease is a leading cause of death in Egypt (not quantified here).

Universal; ghee (samna baladi) is widely used in Egypt.
- Eccezioni individuali
People with familial hypercholesterolaemia or established cardiovascular disease are particularly affected by saturated-fat-induced LDL rises.
- Ambito regionale dell’affermazione
- generale (biologico/medico)

Ghee and butter raise LDL cholesterol relative to unsaturated oils, and LDL causes atherosclerosis; outcome data for butter at modest intakes are weak, so moderate use is a small risk, but the claim that they are not a cause of artery disease is not correct.
Perché alcuni studi pesano di più
RCTs on SFA reduction and LDL causality (I-FAT-01, I-FAT-02), direct lipid RCTs (I-FAT-102, I-FAT-111) and a large butter cohort (I-FAT-101) outweigh null cohort associations of very low certainty (I-FAT-104, I-FAT-103).
È accertato un rapporto causale? Probabile SFA -> LDL is established in RCTs; LDL -> ASCVD is causal; food-level outcome evidence is observational.
Incertezza residua
Ghee-specific outcome data; dose thresholds; matrix effects.
Conclusione giustificata
Unrestricted ghee/butter as the default fat is not supported; plant oils are the better default.
Perché gli studi sembrano in disaccordo?
Dipende dal paese?
- Effetti biologici generali
- Universal.
- Abitudini alimentari
- Samna baladi (ghee) is a traditional default fat in Egypt; hydrogenated vegetable ghee also exists and adds trans-fat risk (I-FAT-103 shows HVO harm).
- Origine dell’alimento
- Ghee fatty-acid profile varies with animal and feed (not quantified).
Ricontrollo statistico
- CI log-symmetry; absolute risk – Symmetric except substitution HR (minor); absolute risks not reported -> ARR not computable. ricalcolato
- LDL unit conversion and CTT-based extrapolation – 7.5 mg/dL = 0.19 mmol/L -> ~5% fewer vascular events if sustained (assumption). ricalcolato
- Consistency of CI and P value for HVO – CI 1.02-2.78 excludes 1 but P=0.058 -> inconsistent; borderline. ricalcolato
Certezza dell’insieme delle prove (GRADE, semplificato)
- Esito principale
- Cardiovascular events with SFA-rich fats v unsaturated oils
- Livello di partenza
- alta (studi randomizzati)
- Motivi di declassamento
- indirectness (few ghee-specific outcome data)
- Risultato
- moderata

New direct evidence on ghee (I-FAT-102) and butter (I-FAT-101, I-FAT-111) supports the existing verdict; a regional null cohort (I-FAT-103) and a cohort meta-analysis (I-FAT-104) are acknowledged as the best supporting evidence but are of very low certainty. The 2026 US guideline allows butter as an option but keeps the 10% saturated-fat cap.

In parole semplici
