Penilaian ilmiah
1. Apa yang diklaim?
Antibiotics should never be used.
2. Apa yang terbukti benar?
Many antibiotic prescriptions are unnecessary (A-ABX-19). For acute otitis media in children, antibiotics reduce pain only modestly (NNTB 20 at 2-3 days) and cause side effects (NNTH 14); serious complications were rare in both groups (I-ABX-09). In mild respiratory infections, delayed or no antibiotics gave similar outcomes for many symptoms with no difference in complications, and cut antibiotic use from 93% to 30% (I-ABX-101). So 'often not needed' is correct for some mild infections.
3. Apa yang salah atau dilebih-lebihkan?
A blanket refusal is contrary to strong evidence. In septic shock, each hour of delay in effective antimicrobial therapy was associated with an average 7.6% decrease in survival (I-ABX-08); in 49,331 sepsis patients, each hour of delay increased risk-adjusted mortality (OR 1.04 per hour) (I-ABX-07); a meta-analysis found lower mortality with antibiotics within 1 hour in septic shock (I-ABX-103). In randomised trials in young children with mild (fast-breathing) pneumonia, amoxicillin reduced treatment failure compared with no antibiotic (RR 0.84, 0.75-0.94; I-ABX-102). The Surviving Sepsis Campaign (I-ABX-104), NICE (sepsis in adults and children, meningitis, pneumonia) and WHO (I-ABX-105) all require prompt antibiotics for serious bacterial infections. The AMR burden studies (A-ABX-18, I-ABX-01) count deaths that occur because antibiotics no longer work, and the GBD authors estimate that better care including 'improved access to antibiotics' could avert 92.0 million deaths between 2025 and 2050 (I-ABX-01). Even the archive that hosts his material states that antibiotics 'save lives'.
4. Apa yang masih belum jelas?
None regarding the core point.
5. Studi mana yang mendukungnya?
6. Studi mana yang membantahnya?
I-ABX-07, I-ABX-08, I-ABX-102 (children, RCTs), I-ABX-103, I-ABX-104 (Surviving Sepsis 2021), I-ABX-105 (WHO AWaRe), I-ABX-01, A-ABX-18; NICE NG253, NG254, NG240, NG250.
7. Apa yang berlaku untuk Jerman dan Eropa?
Standard care in Germany/EU and the UK: antibiotics within 1 hour for high-risk sepsis in adults and children (NICE NG253/NG254) and for suspected bacterial meningitis (NICE NG240), within 4 hours for community-acquired pneumonia in hospital (NICE NG250); stewardship to avoid unnecessary use.
8. Apa yang diketahui tentang Mesir?
Same medical standard; overuse is common in Egyptian private primary care (antibiotics in 81.2% of infection consultations; I-ABX-131) and the regional AMR burden is high (I-ABX-02), which makes appropriate use more, not less, important.
Rincian penilaian
- Alasan tingkat kepastian bukti
Strong, consistent evidence that timely antibiotics reduce mortality in serious bacterial infections (sepsis/septic shock), plus the large burden of deaths from resistant (i.e. effectively untreatable) infections. It is correct that many minor infections (e.g. acute otitis media in high-income settings) often resolve without antibiotics.

Refusing antibiotics in all cases is contrary to strong, consistent evidence and to every guideline found (WHO, NICE, Surviving Sepsis). The correct part is that many mild respiratory infections, such as colds and many ear and throat infections, resolve without antibiotics and that antibiotics are overused.
Mengapa sebagian studi lebih berbobot
Guidelines and RCT/meta-analytic evidence on serious infections weigh most for the harm question; evidence on mild infections answers a different question (when antibiotics can be avoided safely).
Apakah hubungan sebab-akibat sudah terbukti? Ya, terbukti Antibiotics kill bacteria; untreated bacterial sepsis, meningitis and pneumonia have high mortality; RCTs show benefit even in mild childhood pneumonia, and delay in septic shock is associated with higher mortality.
Ketidakpastian yang tersisa
Exact benefit per hour of delay in sepsis without shock; which children with mild pneumonia can safely go without antibiotics.
Kesimpulan yang dapat dibenarkan
Blanket refusal of antibiotics is unsafe; stewardship (use when needed) is correct.
Mengapa studi-studi tampak saling bertentangan?
No true conflict: trials showing that delayed or no antibiotics are safe concern mild, self-limiting respiratory infections in selected patients (I-ABX-101, I-ABX-09), whereas timing and RCT evidence in sepsis and pneumonia concerns infections where antibiotics prevent death or treatment failure (I-ABX-102-104, I-ABX-07, I-ABX-08). In children, an individual 1-hour antibiotic target was not independently significant in one New York cohort (Evans 2018, JAMA, PMID 30043064, abstract read), but completing the whole 1-hour bundle including antibiotics was associated with lower mortality; this concerns minutes-to-hours timing, not refusal.
Apakah ini bergantung pada negara?
- Efek biologis umum
- Universal biology.
- Kebiasaan makan
- Not applicable (medicine use, not diet).
- Aturan hukum
- Antibiotics are prescription medicines in the EU; non-prescription dispensing is common in Egypt and other LMICs.
- Data pemantauan sebenarnya
- Egypt: antibiotics in 81.2% of infection consultations in private primary care (I-ABX-131) - overuse is real there too, which argues for stewardship, not refusal.
Pemeriksaan ulang statistik
- NNT – Jehan 2020: RD 2.3 points -> NNT 43 (27-111); pooled RR 0.84 (0.75-0.94) log-symmetric. dihitung ulang
- Per-hour odds – OR 1.04 per hour -> about 1.27 after 6 hours (multiplicative approximation). dihitung ulang
Kepastian bukti secara keseluruhan (GRADE, disederhanakan)
- Luaran utama
- Mortality/treatment failure without antibiotics in serious bacterial infections
- Tingkat awal
- tinggi (uji acak)
- Alasan penurunan tingkat
- indirectness for some timing data (comparisons of earlier vs later treatment, not none)
- Alasan peningkatan tingkat
- large effect in untreated serious infection
- consistency across guidelines and designs
- Hasil
- tinggi
