That sleeve brings cancer forward.
Long-term reflux and Barrett's after sleeve are documented (C-BAR-04), so a site-specific oesophageal risk is biologically plausible; in a French national cohort oesophageal cancer after sleeve was rare (about 4 per 100,000 person-years) and not significantly different from bypass (HR 1.60, 0.90-2.5; I-BAR-117). Nordic registry data show higher cancer mortality 15+ years after (mostly older) procedures (I-BAR-104).
Overall cancer incidence and cancer mortality were lower, not higher, after bariatric surgery: SOS cancer death HR 0.77 (A-BAR-05), Adams 60% lower cancer mortality (A-BAR-03), SOS 2007 cancer deaths 29 vs 47 (A-BAR-02), SPLENDID 10-year obesity-related cancer 2.9% vs 4.9% (I-BAR-118), upper GI cancer RR 0.58 and oesophageal cancer RR 0.63 (I-BAR-119). Nothing supports cancer appearing "decades earlier".
Sleeve-specific long-term cancer data are limited.
None directly. Limiting: I-BAR-104 (late cancer mortality HR 1.20, older procedures); I-BAR-117 (oesophageal cancer after sleeve vs bypass HR 1.60, not significant).
Most key data are European (Sweden SOS/SOReg, Finland SLEEVEPASS, Switzerland SM-BOSS). European and international society guidelines consider bariatric surgery for severe obesity with lifelong follow-up.
No Egypt-specific outcome data were identified in this review. Elawady states in a transcript that sleeve gastrectomy is very widely performed in Egypt ('مصر كلها بتعملها'); the procedure-specific risks (reflux, deficiencies) apply wherever it is performed, and follow-up quality (supplements, endoscopy, mental-health screening) determines part of the risk.
Several large matched cohorts and meta-analyses consistently show lower cancer incidence and cancer mortality after bariatric surgery, including cohorts with many sleeve operations (SPLENDID I-BAR-118: obesity-related cancer aHR 0.68, cancer death aHR 0.52; I-BAR-102: sleeve cancer death HR 0.26 at 3 years) and specifically for oesophageal cancer (meta-analysis RR 0.63, I-BAR-119). Evidence is observational; a late cancer-mortality signal in Nordic registry data (HR 1.20 at >=15 years, mostly older procedures, I-BAR-104) and the long-term oesophageal risk after sleeve remain unresolved, hence not FALSE/high.

Cancer incidence and cancer mortality are lower, not higher, after bariatric surgery including cohorts with many sleeve operations; a possible small long-term oesophageal risk after sleeve is not excluded.
Several large matched cohorts and meta-analyses with consistent direction; sleeve-specific data (I-BAR-102 cancer death HR 0.26 at 3 y) included.
Consistent reduction with weight loss; observational.
Oesophageal adenocarcinoma after decades of sleeve-related reflux; late cancer-mortality signal in Nordic data.
The claim that sleeve brings cancer decades forward is not supported and is contradicted for cancer overall.
The late Nordic signal (HR 1.20 at >=15 y) concerns mostly older procedures and may reflect confounding; it does not match the claim of cancer decades earlier, and other cohorts show reductions.

Verdict MOSTLY_FALSE kept; certainty raised from low to moderate because new large matched cohorts (I-BAR-118, I-BAR-102) and a 2025 meta-analysis (I-BAR-119) consistently show lower cancer incidence/mortality after surgery, including cohorts with many sleeve operations and specifically oesophageal cancer. MOSTLY_FALSE rather than FALSE because the long-term oesophageal risk after sleeve and a late Nordic cancer-mortality signal remain unresolved.
