1. ¿Qué se afirmó?
Diabetes drugs starve the body by removing glucose from blood and cells.
2. ¿Qué es demostrablemente correcto?
Insulin and sulfonylureas can cause hypoglycaemia, which deprives the brain of glucose; this is the main harm of over-treatment (A-INS-14, A-INS-15, A-INS-16). Trials of intensive glucose lowering roughly doubled to tripled severe hypoglycaemia (HR 2.48, 1.91-3.21) without reducing mortality (I-ODD-108), and in ACCORD targeting near-normal HbA1c in high-risk type 2 diabetes increased mortality (257 vs 203 deaths; HR 1.22, 1.01-1.46) (A-INS-01). This is why guidelines individualise targets (I-INS-10). SGLT2 inhibitors can cause a starvation-like ketotic state in some situations (I-ODD-10).
3. ¿Qué es erróneo o exagerado?
In hyperglycaemia the problem is not lack of glucose in the blood but too much; insulin moves glucose into muscle and fat cells where it is used or stored, so cells are not starved. Insulin deficiency, not insulin, produces an internal 'famine' with breakdown of fat and muscle (I-INS-08).
4. ¿Qué sigue sin estar claro?
No study has tested an 'internal famine' concept because it is not a defined physiological state. The archive's wording ('internal famine') was found in Elawady's transcripts only in other contexts (fasting, war), so it is uncertain whether he used this image for insulin.
5. ¿Qué estudios lo respaldan?
None for an 'internal famine'. Mixed: A-INS-14, A-INS-15 (hypoglycaemia kernel only; hypoglycaemia uncommon and mostly a marker of vulnerability), I-ODD-10.
6. ¿Qué estudios lo contradicen?
I-INS-08 (insulin deficiency causes the catabolic state), I-INS-03 and I-INS-104 (intensive insulin therapy in type 1 diabetes lowered mortality over 27 years: HR 0.67, 0.46-0.99), I-INS-134 (NICE NG17), I-ODD-16 (ADA 2026: insulin essential in type 1 diabetes).
7. ¿Qué se aplica a Alemania y Europa?
European (EASD) and German (DDG) guidance follow the same evidence base as the ADA/EASD consensus: individualised HbA1c targets, insulin as mandatory therapy in type 1 diabetes, and SGLT2 inhibitors/GLP-1 receptor agonists for people with cardiovascular or kidney disease. No country-specific difference changes the assessment.
8. ¿Qué se sabe sobre Egipto?
Egypt has one of the highest diabetes prevalences in the region; the same drug classes and insulins are used. Access barriers and cost may make advice to stop medication attractive, which increases the practical risk of this claim. No Egypt-specific evidence changes the assessment.
Glucose-lowering drugs act by increasing glucose uptake into tissues (insulin), reducing hepatic glucose output (metformin), or excreting excess glucose (SGLT2). Only overdose/hypoglycaemia deprives the brain of glucose. The 'famine' concept has no empirical support apart from that kernel.

Insulin moves glucose from the blood into muscle and fat cells; it does not pull glucose out of cells. The starvation-like state ('famine') in diabetes is caused by insulin deficiency, which leads to DKA. The true kernel is that excessive glucose lowering, mainly with insulin or sulfonylureas, can cause hypoglycaemia and, in ACCORD, aggressive near-normal targets increased mortality; this is why targets are individualised.
Por qué algunos estudios tienen más peso
Physiology and RCTs (DCCT/EDIC, ACCORD, meta-analysis) outweigh the image of 'famine', which has no empirical basis beyond hypoglycaemia.
¿Está demostrada una relación causal? Sí, demostrada Causality is established for hypoglycaemia from overdosing and for harm from over-intensive targets in high-risk patients; a general cellular famine from glucose-lowering drugs is incompatible with established physiology.
Incertidumbre restante
Mechanisms of excess mortality in ACCORD.
Conclusión justificada
The claim is mostly false; only over-treatment causing hypoglycaemia is a real, managed risk. Stopping insulin is dangerous.
¿Por qué parecen contradecirse los estudios?
ACCORD (harm) and DCCT/EDIC or UKPDS follow-up (benefit) answer different questions: ACCORD tested very aggressive targets in older high-risk type 2 patients with older drugs; DCCT/UKPDS tested moderate intensification earlier in disease. Both support individualised targets, neither supports avoiding glucose-lowering treatment.

New evidence (DCCT/EDIC mortality, intensive-control meta-analysis, NICE NG17) is consistent with the existing assessment. ACCORD is added explicitly as the strongest evidence for the hypoglycaemia/over-treatment kernel, which is why the verdict stays MOSTLY_FALSE rather than FALSE.

Does targeting a normal HbA1c (<6.0%) reduce cardiovascular events in type 2 diabetes with high cardiovascular risk?
- ¿Cómo se realizó el estudio?
Multicentre randomised controlled trial (open-label treatment strategies), mean 3.5 years of intensive therapy
- ¿Cuántas personas, animales o muestras se estudiaron?
10,251; Type 2 diabetes, mean age 62.2, median HbA1c 8.1%, established CVD or additional risk factors
- ¿Qué se encontró realmente?
More deaths (HR 1.22) and more severe hypoglycaemia with intensive therapy; primary CV outcome not significantly reduced.
- ¿Qué magnitud tuvo el efecto observado?
HR 1.22 (1.01-1.46) all-cause death; HR 0.90 (0.78-1.04) primary outcome
Very strict sugar targets are not good for everyone, especially older people with heart disease.
- ¿Cuáles son las limitaciones del estudio?
Strategy trial: cannot attribute harm to a specific drug; Intensive arm stopped early; High-risk population; not generalisable to early diabetes
Core evidence that near-normal HbA1c targets can harm high-risk type 2 patients (C-INS-04).
Multinational/US/UK/European data; directly transferable to Germany/EU clinical practice.
That insulin specifically caused the deaths, or that lowering high glucose to moderate targets is harmful.
-
Very tight sugar control: fewer heart attacks, more severe low sugars
Control Group (Turnbull FM et al.). Diabetologia 2009 · PMID 19655124 · DOI 10.1007/s00125-009-1470-0Metaanálisisverificación independienteresumen revisado★ estudio claveMI -15%, major hypoglycaemia about 2.5 times more frequent; death unchanged.
Confirms the hypoglycaemia kernel; does not support a general cellular 'famine'.

El estudio explicado de forma sencilla
- ¿Qué querían averiguar los investigadores?
Pool trials of intensive vs standard glucose control.
- ¿Cómo se realizó el estudio?
Pre-planned meta-analysis.
- ¿Cuántas personas, animales o muestras se estudiaron?
27,049 people.
- ¿Qué se encontró realmente?
Modest MACE reduction; more severe hypoglycaemia; no mortality change.
- ¿Qué magnitud tuvo el efecto observado?
MACE HR 0.91; hypoglycaemia HR 2.48.
Over-treating sugar has a real cost; targets should be individual.
- ¿Cuáles son las limitaciones del estudio?
Older drug strategies.
Supports only the hypoglycaemia kernel.
That glucose-lowering drugs starve cells in general.

Contexto regional
Alemania / UE
European (EASD) and German (DDG) guidance follow the same evidence base as the ADA/EASD consensus: individualised HbA1c targets, insulin as mandatory therapy in type 1 diabetes, and SGLT2 inhibitors/GLP-1 receptor agonists for people with cardiovascular or kidney disease. No country-specific difference changes the assessment.
Egipto
Egypt has one of the highest diabetes prevalences in the region; the same drug classes and insulins are used. Access barriers and cost may make advice to stop medication attractive, which increases the practical risk of this claim. No Egypt-specific evidence changes the assessment.
