Bilimsel değerlendirme
Büyük ölçüde yanlış – İfadenin temel kısımları kanıtlarla desteklenmiyor veya büyük ölçüde abartılmış.
Kanıtın kesinliği: orta – Yeni araştırmalar değerlendirmeyi yine de etkileyebilir.
1. Ne iddia edildi?
PPIs create an inescapable dependence.
2. Kanıtlanabilir şekilde doğru olan ne?
Rebound symptoms can make stopping harder and may contribute to unnecessary long-term use (a hypothesis stated in Lødrup 2013).
3. Yanlış veya abartılı olan ne?
'Cannot stop' is not supported: guidelines (Farrell 2017; AGA 2022) describe successful dose reduction, on-demand use and discontinuation in suitable patients, by tapering or abrupt stop. 'Addiction' is not the right concept: there is no craving or compulsive use.
4. Belirsiz kalan ne?
How often rebound is the main reason for long-term use in practice is not known.
5. Hangi çalışmalar destekliyor?
A-PPI-14 and Niklasson 2010 (I-PPI-104): rebound symptoms after withdrawal (mechanism only); Namikawa 2024 (I-PPI-105): rebound may lead patients to restart PPIs unnecessarily; Cochrane 2017 (I-PPI-103): more symptoms with on-demand use in mild GERD.
6. Hangi çalışmalar çelişiyor?
Koggel 2026 RCT (I-PPI-106): 86% vs 64% discontinued within 2 months; Cochrane 2017 (I-PPI-103): pill burden -3.79 pills/week; Farrell 2017 (I-PPI-05), AGA 2022 (I-PPI-04), Lodrup 2013 (I-PPI-06).
7. Almanya ve Avrupa için ne geçerli?
PPIs are among the most widely prescribed drugs in Germany and Europe; several (e.g. pantoprazole, omeprazole) are sold over the counter in small packs for short-term use. European data (Stockholm, UK GPRD/CPRD, Rotterdam) are among the cited cohorts, so the observational findings apply to European users with the same confounding limits.
8. Mısır hakkında ne biliniyor?
Elawady describes PPIs (Controloc, Nexium) as widely used and sold without prescription in Egypt. No Egypt-specific outcome data were reviewed. The biology and trial results are not expected to differ by country; H. pylori infection, which is common in many countries, is an additional reason why acid-suppression and eradication therapy are prescribed.
9. Hangi pratik sonuç uygundur?
Bir PPI'ı bırakmak isteyenler bunu doktorlarıyla birlikte yapmalı ve birkaç haftalık olası rebound belirtilerini beklemelidir; Barrett özofagusu, ağır özofajiti veya kanama riski olanlar uzman tavsiyesi olmadan bırakmamalıdır.
Değerlendirme ayrıntıları
- Kanıtın kesinliğine ilişkin gerekçe
Rebound is real but transient (two placebo-controlled RCTs: A-PPI-14, I-PPI-104; symptoms gone by week 3 in I-PPI-104). A 2026 randomised trial in outpatients without a chronic indication found that 64% (brochure) to 86% (app) had stopped PPIs after 2 months (I-PPI-106), and a Cochrane review found that deprescribing reduces pill burden, although on-demand use increases symptoms in some people with mild GERD (I-PPI-103). Guidelines describe tapering or abrupt discontinuation (I-PPI-04, I-PPI-05). No evidence of pharmacological addiction was found.
- Kaynak kalitesi
Archive editorial extrapolation from one volunteer RCT.
- Bölgesel aktarılabilirlik
High.
- Bireysel istisnalar
Patients with ongoing reflux disease may genuinely need long-term therapy; this is not dependence.

Rebound can make stopping uncomfortable and may sustain unnecessary use, but most people without an indication can stop; there is no addiction in the pharmacological sense.
Neden bazı çalışmalar daha ağır basar
RCT data on discontinuation and Cochrane evidence outweigh narrative concerns.
Nedensel bir ilişki kanıtlanmış mı? Geçerli değil Dependence in the sense of being unable to stop is contradicted by trial data.
Kalan belirsizlik
Long-term relapse after discontinuation.
Gerekçeli sonuç
Overstated.
Çalışmalar neden çelişiyor gibi görünüyor?
Cochrane (more symptoms with on-demand use) vs Koggel (most can stop) answer different questions: symptom control in GERD patients vs discontinuation in people without indication.

Full text read: weight loss recommended for overweight/obese patients (strong, moderate evidence); avoid meals within 2-3 h of bedtime, avoid tobacco, avoid 'trigger foods', elevate head of bed for nocturnal symptoms (conditional, low evidence); Table 3 rates 'avoid carbonated beverages' as moderate evidence and recommendable; PPIs over H2RAs for healing erosive oesophagitis (strong, high evidence); attempt to discontinue PPIs in patients without erosive oesophagitis or Barrett's whose symptoms resolved (conditional, low); maintenance PPI indefinitely or surgery for LA grade C/D oesophagitis (strong, moderate); endoscopy first for dysphagia or alarm symptoms (weight loss, GI bleeding); rebound shown in healthy controls but strong evidence for symptom increase after abrupt withdrawal in patients is lacking.
