Intensive glucose lowering in type 2 diabetes increased mortality (ACCORD) and severe hypoglycaemia is linked to death.
ACCORD: 10,251 patients, target HbA1c <6.0%: 257 vs 203 deaths, HR 1.22 (95% CI 1.01-1.46); the intensive arm was stopped after 3.5 years; no significant reduction of the primary cardiovascular outcome (HR 0.90) (A-INS-01). Full text reports death from cardiovascular causes HR 1.35 (1.04-1.76), so the archive's 35% figure is correct. VADT found no benefit on major cardiovascular events or death (HR 1.07) and more adverse events, mainly hypoglycaemia (A-INS-03). Severe hypoglycaemia was associated with higher mortality in ACCORD (adjusted HR 1.41 intensive, 2.30 standard arm) (A-INS-14) and with macrovascular events (HR 2.88) and death (HR 2.69) in ADVANCE (A-INS-15). In critically ill patients, tight glucose control (4.5-6.0 mmol/L) increased 90-day mortality (A-INS-02). ADA 2026 states that clinicians 'should not aggressively attempt to achieve near-normal A1C levels' where this cannot be done safely (I-INS-10).
The archive's Arabic headline attributes the ACCORD harm to 'high insulin doses'; ACCORD tested a treatment strategy using many drugs (in the intensive arm 94.7% metformin, 91.7% thiazolidinedione, 86.6% secretagogue and 77.3% any insulin, per full text) and could not attribute the excess mortality to any single drug. Bonds et al. explicitly concluded that hypoglycaemia did not appear to account for the mortality difference between arms (A-INS-14). ADVANCE (target HbA1c <=6.5%) found a 10% reduction in combined major macro- and microvascular events, mainly nephropathy, and no increase in death (HR 0.93) (I-INS-01). UKPDS showed microvascular benefit (A-INS-16) and long-term reductions of myocardial infarction and death (I-INS-02); in type 1 diabetes intensive insulin therapy reduced retinopathy by 76% (I-INS-03). The archive's statement that severe hypoglycaemia doubled mortality 'in both arms' is correct only for the standard arm (adjusted HR 2.30); in the intensive arm the adjusted HR was 1.41 (A-INS-14).
The exact mechanism of excess death in ACCORD remains unexplained. In 9-year follow-up (ACCORDION) the excess of cardiovascular death persisted but was smaller (HR 1.20, 1.03-1.39), while all-cause mortality became neutral (I-INS-123).
European (EASD) and German (DDG) guidance follow the same evidence base as the ADA/EASD consensus: individualised HbA1c targets, insulin as mandatory therapy in type 1 diabetes, and SGLT2 inhibitors/GLP-1 receptor agonists for people with cardiovascular or kidney disease. No country-specific difference changes the assessment.
Egypt has one of the highest diabetes prevalences in the region; the same drug classes and insulins are used. Access barriers and cost may make advice to stop medication attractive, which increases the practical risk of this claim. No Egypt-specific evidence changes the assessment.
Directly reported RCT results (ACCORD abstract; cardiovascular death HR 1.35 confirmed in full text). Consistent with VADT, ADVANCE and current guidelines. Downgraded from TRUE because the archive omits that ADVANCE showed microvascular benefit, that hypoglycaemia did not explain the ACCORD mortality excess, and that the harm concerned a near-normal target in high-risk patients, not glucose lowering in general.
Archive editorial correctly quoting RCTs.
Trials were multinational (US/Canada, ADVANCE incl. Europe/Asia); transferable.
Younger people early in the disease without cardiovascular disease may benefit from lower targets if achieved safely (ADA 2026, I-INS-10).

ACCORDION confirms a persistent CV-death excess while total mortality became neutral; this refines but does not change the verdict. Certainty for the claim as a whole (multiple RCTs reporting these figures) remains high even though the mortality outcome alone is inconsistent across trials.
