Bilimsel değerlendirme
Büyük ölçüde yanlış – İfadenin temel kısımları kanıtlarla desteklenmiyor veya büyük ölçüde abartılmış.
Kanıtın kesinliği: yüksek – Yeni araştırmaların değerlendirmeyi değiştirmesi çok olası değildir.
1. Ne iddia edildi?
Insulin is not needed for glucose uptake, so it does not treat anything and starves cells.
2. Kanıtlanabilir şekilde doğru olan ne?
Glucose uptake into red blood cells and brain does not require insulin; glycated haemoglobin forms inside red cells without insulin. Too much insulin causes hypoglycaemia, which deprives the brain of glucose (A-INS-14, A-INS-15).
3. Yanlış veya abartılı olan ne?
Muscle (the largest glucose sink after meals) and fat tissue need insulin for glucose uptake; the liver needs insulin signals to stop releasing glucose and producing ketones. That is why absolute insulin deficiency (type 1 diabetes) causes high glucose, ketoacidosis and weight loss despite eating. HbA1c is a valid measure of average glucose exposure and predicts complications (I-INS-04). Insulin therapy lowers glucose and complications in RCTs (I-INS-03, A-INS-16). The claim that 'no drug on earth lowers sugar' is contradicted by every glucose-lowering trial cited by the archive itself.
4. Belirsiz kalan ne?
When insulin 'does not work' in practice, causes such as injection into lipohypertrophic tissue (blunted and variable absorption, A-INS-20), insulin resistance or omitted doses should be considered; they do not show that insulin is ineffective.
5. Hangi çalışmalar destekliyor?
Physiology of insulin-independent glucose transporters (kernel only).
6. Hangi çalışmalar çelişiyor?
I-INS-03, I-INS-04, A-INS-16, I-INS-07, I-INS-104 (intensive insulin therapy associated with lower long-term mortality), I-INS-116 (insulin withdrawal lowers muscle ATP production and raises ketones), I-INS-133, I-INTB-01.
7. Almanya ve Avrupa için ne geçerli?
European (EASD) and German (DDG) guidance follow the same evidence base as the ADA/EASD consensus: individualised HbA1c targets, insulin as mandatory therapy in type 1 diabetes, and SGLT2 inhibitors/GLP-1 receptor agonists for people with cardiovascular or kidney disease. No country-specific difference changes the assessment.
8. Mısır hakkında ne biliniyor?
Egypt has one of the highest diabetes prevalences in the region; the same drug classes and insulins are used. Access barriers and cost may make advice to stop medication attractive, which increases the practical risk of this claim. No Egypt-specific evidence changes the assessment.
9. Hangi pratik sonuç uygundur?
Bazı hücrelerin glukozu insülinsiz alması insülini gereksiz kılmaz; insüline ihtiyacı olan kişiler onu kullanmaya devam etmelidir.
Değerlendirme ayrıntıları
- Kanıtın kesinliğine ilişkin gerekçe
Correct kernel: red blood cells, brain and liver take up glucose largely independent of insulin (GLUT1/GLUT3/GLUT2), and HbA1c forms inside red cells without insulin. Wrong conclusion: skeletal muscle and fat depend on insulin (GLUT4); insulin also suppresses liver glucose and ketone production; insulin deficiency leads to ketoacidosis and death (type 1 diabetes). Insulin's efficacy is shown in RCTs (DCCT, UKPDS).

Kanıtların bütünü ne söylüyor?
Yukarıdaki çalışmalara dayalı editoryal değerlendirme – kendisi bir çalışma sonucu değildir.
Hangi görüş daha iyi destekleniyor?
Some tissues (brain, red cells) take up glucose without insulin, but muscle and fat need insulin, and insulin suppresses hepatic glucose and ketone output; insulin therapy lowers complications and mortality.
Neden bazı çalışmalar daha ağır basar
RCTs and physiology outweigh the correct but misapplied kernel.
Nedensel bir ilişki kanıtlanmış mı? Evet, kanıtlanmış Insulin deficiency causes hyperglycaemia and ketoacidosis; insulin reverses them.
Kalan belirsizlik
None material.

Mostly false: true kernel, wrong conclusion.

Multinational/US/UK/European data; directly transferable to Germany/EU clinical practice.
- Bu çalışmadan açıkça hangi sonuç çıkarılamaz?
That hypoglycaemia caused the ACCORD excess mortality.

Çalışmanın basit açıklaması
- Araştırmacılar neyi öğrenmek istedi?
Severe hypoglycaemia and vascular events/death
- Çalışma nasıl yapıldı?
Post-hoc cohort analysis of an RCT (ADVANCE)
- Kaç kişi, hayvan veya numune incelendi?
11,140; ADVANCE participants with type 2 diabetes
- Gerçekte ne bulundu?
aHR 2.69 death.
- Gözlenen etki ne kadar büyüktü?
aHR 2.69 (1.97-3.67)
- Sonuçlar günlük yaşamda ne anlama geliyor?
Avoid severe hypoglycaemia.
- Çalışmanın sınırlılıkları neler?
Only 231 people had severe hypoglycaemia; links also seen with skin and digestive disease (suggesting frailty); no dose-response; intensive treatment was a sulfonylurea, not insulin; funded by the drug's manufacturer.

Çalışmanın basit açıklaması
- Araştırmacılar neyi öğrenmek istedi?
Insulin absorption from lipohypertrophic tissue
- Çalışma nasıl yapıldı?
Randomised crossover clamp and meal study
- Kaç kişi, hayvan veya numune incelendi?
13; Type 1 diabetes with lipohypertrophy
- Gerçekte ne bulundu?
Blunted, variable absorption.
- Gözlenen etki ne kadar büyüktü?
Insulin AUC -21%
- Sonuçlar günlük yaşamda ne anlama geliyor?
Rotate injection sites.
- Çalışmanın sınırlılıkları neler?
Small n
- Elawady'nin somut iddiası hakkında ne söylüyor?
Peripheral; shows correct injection technique matters.
- Sonuçlar Almanya ve Avrupa için geçerli mi?
German study.
- Bu çalışmadan açıkça hangi sonuç çıkarılamaz?
That insulin is harmful.

Bölgesel bağlam
Almanya / AB
European (EASD) and German (DDG) guidance follow the same evidence base as the ADA/EASD consensus: individualised HbA1c targets, insulin as mandatory therapy in type 1 diabetes, and SGLT2 inhibitors/GLP-1 receptor agonists for people with cardiovascular or kidney disease. No country-specific difference changes the assessment.
Mısır
Egypt has one of the highest diabetes prevalences in the region; the same drug classes and insulins are used. Access barriers and cost may make advice to stop medication attractive, which increases the practical risk of this claim. No Egypt-specific evidence changes the assessment.

Genel bilimsel değerlendirme
Büyük ölçüde yanlış Bazı hücreler glukozu insülinsiz alır, ancak bu insülin tedavisini yararsız veya zararlı yapmaz.
