Valutazione scientifica
1. Che cosa è stato affermato?
Insulin itself doubles mortality and raises cancer and complications in a dose-dependent way.
2. Che cosa è dimostrabilmente corretto?
The figures are quoted correctly as associations: insulin monotherapy vs metformin aHR 2.197 for death, 1.437 for cancer (A-INS-08; this study did not analyse insulin dose); per 1-unit increase in IU/kg/day aHR 1.54 for death (A-INS-10); adding insulin vs sulfonylurea to metformin aHR 1.30 for the composite and 1.44 for death (A-INS-13); insulin use and cancer mortality HR 1.9 (A-INS-11) with a gradient by number of pharmacy dispensations (A-INS-12; cancer mortality only, called preliminary by its authors); insulin-based regimens vs combination oral agents HR 1.49 (A-INS-09).
3. Che cosa è sbagliato o esagerato?
All of these are retrospective observational studies. In Currie et al. insulin users differed sharply at baseline (diabetes duration 5.2 vs 1.5 years; creatinine >130 umol/L in 21.1% vs 2.2%), follow-up was censored at regimen switch, and bias is judged critical; the study cannot support a causal 'doubling' or the dose-response element (A-INS-08). In Bowker et al. the 'dose' was the number of dispensations counted in the same window as death, which invites reverse causation (A-INS-12). Holden et al. state the association should be tested in interventional studies (A-INS-10); Bowker et al. state it is uncertain whether the risk reflects harm from insulin, protection by metformin, or unmeasured factors (A-INS-11); Roumie et al. call for further investigation (A-INS-13). Patients receive insulin, and higher doses, because their disease is more advanced and they are more insulin resistant, obese or ill (confounding by indication; reverse causation). A meta-analysis of 20 RCTs found no mortality effect of insulin (RR 0.99, 0.92-1.06), but more severe hypoglycaemia (RR 1.70) (I-INS-122). In the randomised ORIGIN trial, glargine had neutral effects on cardiovascular outcomes (HR 1.02) and cancers (HR 1.00) over 6.2 years (A-INS-07, I-INS-05), and in UKPDS insulin-based intensive therapy reduced microvascular complications without excess macrovascular disease (A-INS-16) and was associated with fewer deaths at 10 years (I-INS-02).
4. Che cosa resta non chiaro?
RCT evidence in very insulin-resistant, high-dose, late-stage type 2 diabetes is limited; whether high insulin doses carry some independent risk cannot be excluded.
5. Quali studi la supportano?
A-INS-08, A-INS-09, A-INS-10, A-INS-11, A-INS-12, A-INS-13 (associations only; the IU/kg/day dose-response comes from A-INS-10 alone).
6. Quali studi la contraddicono?
European (EASD) and German (DDG) guidance follow the same evidence base as the ADA/EASD consensus: individualised HbA1c targets, insulin as mandatory therapy in type 1 diabetes, and SGLT2 inhibitors/GLP-1 receptor agonists for people with cardiovascular or kidney disease. No country-specific difference changes the assessment.
8. Che cosa si sa sull’Egitto?
Egypt has one of the highest diabetes prevalences in the region; the same drug classes and insulins are used. Access barriers and cost may make advice to stop medication attractive, which increases the practical risk of this claim. No Egypt-specific evidence changes the assessment.
The numbers are correctly reported from observational cohorts, but these are strongly confounded by indication (sicker patients with longer disease and failure of tablets receive insulin; doses rise with insulin resistance and illness); the authors themselves call for interventional studies. A meta-analysis of 20 RCTs (18,599 patients) found no effect of insulin on all-cause mortality (RR 0.99, 0.92-1.06) or cardiovascular mortality (RR 0.99) versus other drugs (I-INS-122), and ORIGIN and UKPDS found no increase in mortality, cardiovascular events or cancer. Certainty raised from low to moderate because randomised evidence directly excludes a doubling of mortality; it is not high because late-stage, high-dose users are under-represented in RCTs.

Randomised evidence shows no mortality increase with insulin in T2D; the observational associations are explained largely by confounding by indication and reverse causation.
Perché alcuni studi pesano di più
RCT meta-analysis directly excludes a doubling; retrospective database studies are at critical risk of bias.
È accertato un rapporto causale? Non accertato No randomised evidence of harm; observational signals confounded.
Incertezza residua
Late-stage, high-dose, very insulin-resistant patients are under-represented in RCTs.
Conclusione giustificata
Mostly false.
Perché gli studi sembrano in disaccordo?
Observational cohorts (higher mortality) vs RCTs (neutral): different patient mix (insulin given to sicker patients) and analytic biases explain the discrepancy.
Dipende dal paese?
- Effetti biologici generali
- Insulin physiology, autoimmune type 1 diabetes and ketoacidosis are universal human biology.
- Abitudini alimentari
- Egyptian and European diets differ, but no dietary pattern replaces insulin in insulin-deficient diabetes.
- Origine dell’alimento
- Not relevant.
- Contaminanti
- Not relevant.
- Condizioni di produzione nazionali
- Not relevant.
- Norme giuridiche
- Insulin is an essential medicine (WHO list) in Egypt and the EU; prescription rules not relevant to the scientific question.
- Dati di monitoraggio effettivi
- Egyptian/Middle Eastern data show severe DKA is common at presentation in children (I-INS-115, I-INS-114); no data on outcomes among Tayebat followers.
Ricontrollo statistico
- Is a doubling of mortality compatible with RCT CIs? – No: all-cause RR 0.99 (0.92-1.06); RR 2.0 lies far outside ricalcolato
Certezza dell’insieme delle prove (GRADE, semplificato)
- Esito principale
- All-cause mortality with insulin therapy in T2D
- Livello di partenza
- alta (studi randomizzati)
- Motivi di declassamento

Verdict unchanged. Certainty raised from low to moderate because an RCT meta-analysis (20 trials, 18,599 patients) directly tests the mortality question and excludes a doubling (RR 0.99, 0.92-1.06); the remaining indirectness concerns late-stage, high-dose users.

In parole semplici

Contesto regionale
Germania / UE
European (EASD) and German (DDG) guidance follow the same evidence base as the ADA/EASD consensus: individualised HbA1c targets, insulin as mandatory therapy in type 1 diabetes, and SGLT2 inhibitors/GLP-1 receptor agonists for people with cardiovascular or kidney disease. No country-specific difference changes the assessment.
Egitto
Egypt has one of the highest diabetes prevalences in the region; the same drug classes and insulins are used. Access barriers and cost may make advice to stop medication attractive, which increases the practical risk of this claim. No Egypt-specific evidence changes the assessment.
Che cosa vale nel mio paese?
Valutazione scientifica generale
